Summer Surgery Gift

Effective Date: June 16, 2026

The “Summer Surgery Gift” is a limited-time appreciation benefit offered exclusively to selected patients of New Face MD Surgery Centers LLC who have already received a surgical quotation, paid a surgical deposit, have not yet scheduled their surgical procedure, and have received an official physical postcard invitation mailed directly to their home by New Face MD Surgery Centers LLC.

Eligible patients who meet all requirements outlined in these Terms and Conditions may receive three (3) complimentary post-surgical massages and one (1) complimentary hyperbaric oxygen therapy session, with a combined commercial value of Four Hundred Fifty Dollars (US $450.00), provided that the patient schedules and completes their qualifying surgical procedure on or before August 30, 2026.

This promotional benefit is available only to selected patients who present the original physical postcard at the time of redemption and who comply with all medical, administrative, financial, and scheduling requirements established by New Face MD Surgery Centers LLC.

Eligibility, limitations, and conditions of this benefit are governed exclusively by the Terms and Conditions outlined below.

TERMS & CONDITIONS – SUMMER SURGERY GIFT – New Face MD Surgery Centers LLC

These Terms and Conditions (“Terms”) constitute a legally binding agreement between the patient (“Patient”) and New Face MD Surgery Centers LLC (“Clinic”).

By presenting the official Summer Surgery Gift postcard, scheduling surgery, signing the required documentation, and completing the qualifying surgical procedure within the applicable deadline, the Patient expressly acknowledges, accepts, and agrees to be bound by all provisions contained herein.

1. Definitions

For purposes of these Terms:

“Clinic” refers to New Face MD Surgery Centers LLC, including its affiliated medical facilities, administrative entities, agents, staff members, and authorized medical providers.

“Patient” refers to the individual who has received a surgical quotation from the Clinic, paid a surgical deposit, received the official physical Summer Surgery Gift postcard, and proceeds to schedule and undergo a qualifying surgical procedure under the conditions of this promotion.

“Promotion” or “Summer Surgery Gift” refers exclusively to the limited-time complimentary benefit consisting of three (3) post-surgical massages and one (1) hyperbaric oxygen therapy session offered by the Clinic under the conditions set forth herein.

“Physical Postcard” refers to the official printed Summer Surgery Gift postcard mailed directly by the Clinic to selected Patients at their residential mailing address.

“Qualifying Patient” refers to a Patient who has an existing paid surgical deposit, has received a surgical quotation or has a deposit that remains valid under Clinic policy, has not yet completed the surgical procedure, has received the Physical Postcard, and complies with all terms required to redeem the Promotion.

“Deposit” refers to the non-refundable monetary amount previously paid by the Patient to secure the ability to schedule a surgical procedure with the Clinic under the applicable Invoice of Surgical Fees and Contract.

“Active Deposit” refers to a Deposit that remains valid under the Clinic’s deposit policy. The standard deposit validity period is one (1) year from the date of payment, unless otherwise stated in the Patient’s executed agreement.

“Qualifying Procedure” refers to the surgical procedure previously quoted to the Patient and associated with the Patient’s paid Deposit, provided that the Patient is medically cleared and approved to undergo such procedure by the Clinic’s surgical team.

“Complimentary Benefits” refers collectively to three (3) post-surgical massages and one (1) hyperbaric oxygen therapy session provided by New Face MD Surgery Centers LLC as part of the Summer Surgery Gift.

“Redemption Deadline” refers to August 30, 2026, the final date by which the Patient must both schedule and complete the Qualifying Procedure in order to retain eligibility for the Complimentary Benefits.

“Provider Availability” refers to the professional, medical, operational, and logistical capacity of the Clinic’s surgeons, staff, facilities, and treatment providers to perform the Qualifying Procedure and provide the Complimentary Benefits within the applicable timeframe.

1. Promotional Period and Redemption Deadline

The Summer Surgery Gift is available only to selected Patients who have received the official Physical Postcard and who schedule and complete their Qualifying Procedure on or before August 30, 2026.

August 30, 2026, shall be considered the final eligible date to complete surgery under this Promotion.

To qualify for this Promotion, the Patient must:

1. Have previously paid a surgical Deposit to New Face MD Surgery Centers LLC.
2. Have a valid surgical quotation or an Active Deposit associated with a prior quotation.
3. Have received the official Summer Surgery Gift Physical Postcard mailed by the Clinic.
4. Present the original Physical Postcard to redeem the Complimentary Benefits.
5. Schedule and complete the Qualifying Procedure on or before August 30, 2026.
6. Be medically cleared and administratively approved for surgery.
7. Comply with all payment obligations, contractual requirements, and preoperative instructions established by the Clinic.

This Promotion does not extend, renew, or modify the original validity period of the Patient’s Deposit. If the Patient’s quotation has expired but the Deposit remains valid under Clinic policy, the Patient may still qualify for the Complimentary Benefits, subject to all other eligibility requirements stated herein.

No exceptions, extensions, substitutions, or retroactive applications shall be granted after August 30, 2026.

1. Promotional Benefit

Eligible Patients who meet all requirements under these Terms may receive the following Complimentary Benefits:

• Three (3) post-surgical massages, to be performed exclusively by authorized New Face MD Surgery Centers LLC personnel.
• One (1) hyperbaric oxygen therapy session, to be performed exclusively at New Face MD Surgery Centers LLC facilities.
• The combined commercial value of the Complimentary Benefits is Four Hundred Fifty Dollars (US $450.00).

The Complimentary Benefits are offered solely as a non-monetary promotional gift and shall not be interpreted as a discount, refund, credit, price reduction, or modification of the Patient’s original surgical quotation, contract, invoice, financing agreement, or balance owed.

The Complimentary Benefits carry no cash value and may not be exchanged for money, credit, alternate services, products, procedures, discounts, or future promotional benefits.

1. Patient Eligibility

Participation in the Summer Surgery Gift is limited exclusively to selected Patients who received the official Physical Postcard from New Face MD Surgery Centers LLC.

To qualify, the Patient must:

• Be eighteen (18) years of age or older.
• Have previously paid a surgical Deposit to the Clinic.
• Have a valid Deposit under the Clinic’s deposit policy.
• Have a surgical procedure previously quoted or associated with the paid Deposit.
• Have received the official Summer Surgery Gift Physical Postcard by mail.
• Present the original Physical Postcard to redeem the Complimentary Benefits.
• Schedule and complete the Qualifying Procedure on or before August 30, 2026.
• Complete all required medical evaluations, preoperative requirements, documentation, and payment obligations.
• Be deemed an appropriate surgical candidate based on medical history, physical evaluation, BMI, body composition, surgical complexity, and overall safety considerations.
• Receive medical clearance and final provider approval prior to surgery.

Eligibility is personal, limited, and subject to verification. Receiving a Physical Postcard does not guarantee medical approval, surgical clearance, provider availability, or final eligibility to undergo surgery.

The Clinic reserves the right to determine that a Patient is not eligible for this Promotion at any time if medical, administrative, contractual, financial, or operational requirements are not met.

1. Application to Existing Deposits and Quotations

This Promotion applies only to one (1) qualifying surgical quotation or procedure associated with the Patient’s existing paid Deposit.

If the Patient’s original quotation has expired but the Deposit remains valid under the Clinic’s deposit policy, the Patient may still qualify for the Complimentary Benefits, provided that all other requirements are satisfied and the Qualifying Procedure is completed on or before August 30, 2026.

The Physical Postcard is valid only for the Patient to whom it was mailed and only in connection with one (1) qualifying surgical procedure associated with that Patient’s paid Deposit.

This Promotion does not alter, extend, reinstate, or renew any expired Deposit. If the Deposit has expired or is no longer valid under Clinic policy, the Patient is not eligible for this Promotion unless otherwise expressly approved in writing by the Clinic.

Any pricing, balance, financing terms, or contractual obligations related to the Qualifying Procedure shall remain governed by the Patient’s applicable Invoice of Surgical Fees and Contract and/or financing agreement.

1. Financing

Patients who use third-party financing or payment plans may qualify for the Summer Surgery Gift, provided that they meet all eligibility requirements outlined in these Terms.

Financing approval does not guarantee medical eligibility, surgical approval, provider availability, or promotional eligibility.

All financing terms, balances, payment deadlines, approvals, denials, fees, or obligations remain subject to the policies and requirements of the applicable financing provider and the Patient’s executed agreement with the Clinic.

The Complimentary Benefits do not reduce, replace, or modify any financed amount, surgical balance, payment obligation, or contractual requirement.

1. Scheduling and Availability

All surgical dates, consultations, post-surgical massages, hyperbaric oxygen therapy sessions, providers, and facilities are subject to availability.

To retain eligibility for the Summer Surgery Gift, the Patient must schedule and complete the Qualifying Procedure on or before August 30, 2026.

Scheduling of surgery is subject to:

• Provider availability.
• Operating room capacity.
• Medical clearance.
• Completion of all required preoperative testing and documentation.
• Compliance with payment deadlines.
• Compliance with the Patient’s Invoice of Surgical Fees and Contract.
• Administrative approval by the Clinic.

The Clinic does not guarantee specific surgery dates, providers, appointment times, or treatment availability beyond what is expressly confirmed in writing.

Patients are strongly encouraged to schedule promptly. Availability may become limited or unavailable before August 30, 2026, and the Clinic shall have no obligation to extend the Promotion, provide substitute benefits, or offer financial compensation if the Patient fails to schedule in a timely manner.

1. Redemption of Complimentary Benefits

To redeem the Complimentary Benefits, the Patient must present the original Summer Surgery Gift Physical Postcard provided by New Face MD Surgery Centers LLC.

The Physical Postcard must be presented as instructed by the Clinic and may be required at the time of scheduling, surgery confirmation, or post-surgical benefit redemption.

The Complimentary Benefits may only be redeemed by the Patient whose name is associated with the paid Deposit and qualifying surgical file.

The Complimentary Benefits must be redeemed in accordance with the Clinic’s postoperative care schedule, provider recommendations, and appointment availability.

Post-surgical massages and hyperbaric oxygen therapy must be performed exclusively by New Face MD Surgery Centers LLC personnel and/or at New Face MD Surgery Centers LLC facilities. Benefits may not be performed by third-party providers, transferred to outside vendors, or reimbursed if obtained elsewhere.

Failure to present the original Physical Postcard may result in forfeiture of the Complimentary Benefits.

1. Cancellations, Rescheduling, and Failure to Complete Surgery

The Patient must both schedule and complete the Qualifying Procedure on or before August 30, 2026, in order to retain eligibility for the Complimentary Benefits.

If the Patient cancels the surgery, fails to proceed with surgery, does not obtain medical clearance, fails to comply with payment obligations, or reschedules the procedure to a date after August 30, 2026, the Complimentary Benefits shall be automatically forfeited without refund, credit, substitution, or compensation.

Because the Complimentary Benefits are directly connected to the Patient’s completion of surgery, a Patient who cancels or does not undergo the Qualifying Procedure shall not be entitled to receive the post-surgical massages, hyperbaric oxygen therapy session, or any equivalent value.

All matters relating to cancellation, rescheduling, penalties, refunds, or deposit validity shall remain governed exclusively by the Patient’s signed Invoice of Surgical Fees and Contract.

1. Inclusions

The Summer Surgery Gift includes only the benefits expressly listed below:

• Three (3) post-surgical massages performed by authorized New Face MD Surgery Centers LLC personnel.
• One (1) hyperbaric oxygen therapy session performed at New Face MD Surgery Centers LLC facilities.
• A combined commercial value of Four Hundred Fifty Dollars (US $450.00).

The Complimentary Benefits are subject to medical appropriateness, provider recommendation, scheduling availability, postoperative protocols, and Clinic policies.

The Clinic reserves the right to modify the timing, sequence, or administration of the Complimentary Benefits if medically or operationally necessary.

No inclusion under this Promotion carries cash value, refund entitlement, transferable credit, or exchange value.

1. Exclusions

This Promotion expressly excludes any services, products, procedures, benefits, or conditions not explicitly listed as included under these Terms.

The following exclusions apply without exception:

• Patients who did not receive the official Summer Surgery Gift Physical Postcard.
• Patients who cannot present the original Physical Postcard when required.
• Patients whose Deposit is expired or no longer valid under Clinic policy.
• Patients who do not schedule and complete surgery on or before August 30, 2026.
• Patients who cancel surgery or reschedule surgery to a date after August 30, 2026.
• Patients who fail to meet medical, financial, administrative, or contractual requirements.
• Any service not expressly listed as part of the Complimentary Benefits.
• Additional massages beyond the three (3) included sessions.
• Additional hyperbaric oxygen therapy sessions beyond the one (1) included session.
• Preoperative exams, laboratory testing, imaging studies, medications, garments, postoperative supplies, travel expenses, lodging, transportation, recovery house services, third-party services, or any other cost not expressly included in writing.
• Any benefit, treatment, or service deemed medically inappropriate, unsafe, unnecessary, or contraindicated by the Clinic or its providers.

The Clinic reserves the right to withhold any Complimentary Benefit if, in the professional judgment of the medical team, the treatment is not appropriate for the Patient’s safety, recovery, or clinical condition.

1. No Cash Value

The Complimentary Benefits have a combined commercial value of Four Hundred Fifty Dollars (US $450.00), but they are provided solely as a promotional gift.

The Complimentary Benefits:

• Have no redeemable cash value.
• Are not refundable.
• Are not transferable.
• Are not exchangeable for other services.
• Are not applicable as credit toward surgery.
• May not be applied to balances, deposits, financing payments, cancellation fees, or future procedures.
• May not be substituted for products, medications, garments, appointments, or other treatments.

No unused portion of the Complimentary Benefits shall be refunded, credited, carried forward, transferred, or exchanged.

1. Non-Transferability

The Summer Surgery Gift is personal and non-transferable.

The Physical Postcard, Complimentary Benefits, Deposit, quotation, and promotional eligibility may be redeemed only by the Patient whose name appears in the Clinic’s records and whose paid Deposit is associated with the Qualifying Procedure.

This Promotion cannot be sold, gifted, assigned, transferred, shared, or applied to another individual, regardless of relationship, family status, or intent.

Any attempt to transfer, sell, assign, or misuse the Physical Postcard or promotional eligibility shall automatically void the Promotion without refund, replacement, substitution, or financial compensation.

1. Non-Combinability

The Summer Surgery Gift is valid for one (1) active qualifying quotation or surgical procedure associated with the Patient’s paid Deposit.

This Promotion may not be combined with any other offer, discount, credit, referral incentive, loyalty reward, seasonal special, complimentary service, negotiated benefit, or future promotion unless expressly approved in writing by New Face MD Surgery Centers LLC.

If the Patient qualifies for multiple promotions, only one promotion may apply, as determined by the Clinic in its sole discretion.

The Summer Surgery Gift may not be stacked, prorated, exchanged, converted, or combined for additional value or compensation.

1. Medical Suitability and Disclaimer of Results

All surgical procedures and postoperative treatments are subject to individual variability based on patient anatomy, medical history, healing capacity, compliance with postoperative instructions, and overall health condition.

The inclusion of post-surgical massages and hyperbaric oxygen therapy does not guarantee any specific surgical result, recovery speed, healing outcome, appearance, or aesthetic improvement.

Patients acknowledge and accept that:

• Surgical and recovery outcomes vary by individual.
• Postoperative treatments are supportive services and do not guarantee a specific result.
• The Clinic may withhold, delay, modify, or decline a Complimentary Benefit if medically appropriate.
• Any additional treatments, revisions, corrections, or services requested after surgery shall be subject to separate fees unless otherwise stated in writing.

No verbal assurances, marketing statements, or informal communications shall be interpreted as a guarantee of surgical outcome, healing result, or promotional entitlement beyond what is expressly stated in these Terms.

1. Modification or Cancellation of Promotion

New Face MD Surgery Centers LLC reserves the right to modify, suspend, limit, or terminate the Summer Surgery Gift at any time and without prior notice, for any reason deemed necessary by management, including but not limited to provider availability, clinical capacity, operational limitations, safety concerns, administrative needs, or regulatory requirements.

Any modification or cancellation shall not entitle the Patient to compensation, refund, substitute benefit, or financial equivalent.

Patients who have fully complied with all eligibility requirements, presented the Physical Postcard, scheduled surgery, completed surgery on or before August 30, 2026, and satisfied all contractual obligations will have their Complimentary Benefits honored under the Terms in effect at the time of qualification, subject to medical and operational availability.

No exceptions or extensions shall be made after August 30, 2026.

1. Governing Law and Jurisdiction

These Terms and Conditions, and any dispute or claim arising from or related to the Summer Surgery Gift, shall be governed by and construed in accordance with the laws of the State of Florida, without regard to conflict-of-law principles.

The parties expressly agree that exclusive jurisdiction and venue shall rest with the state and federal courts located in Miami-Dade County, Florida, and the Patient irrevocably submits to the personal jurisdiction of those courts for the resolution of any claim or dispute.

Should any provision of these Terms be deemed invalid or unenforceable, the remaining provisions shall remain in full force and effect.

No waiver by the Clinic of any breach or default shall be deemed a waiver of any subsequent breach or default.

The English version of these Terms and Conditions shall be the only legally binding version. Any translated copies are provided solely for convenience and shall not alter the legal interpretation of this document.

1. Acceptance of Terms

By presenting the Summer Surgery Gift Physical Postcard, scheduling surgery, signing the required documentation, and/or completing the Qualifying Procedure within the applicable deadline, the Patient acknowledges, accepts, and agrees to be legally bound by all provisions contained in these Terms and Conditions.

Such acceptance constitutes a full and unconditional understanding that:

• The Summer Surgery Gift is a limited-time, non-monetary promotional benefit.
• The Complimentary Benefits are available only to selected Patients who received the official Physical Postcard.
• The Patient must complete the Qualifying Procedure on or before August 30, 2026.
• The Complimentary Benefits have no cash value and are not refundable, transferable, or exchangeable.
• The Promotion does not extend or modify the original validity period of the Patient’s Deposit.
• No verbal statement, marketing material, informal message, or conversation shall modify or supersede the written terms of this document.

The Clinic encourages all Patients to ask any questions regarding eligibility, scheduling, or redemption prior to confirming surgery.
