BOOTOX TERMS AND CONDITIONS

  1. The “Bootox spin to win game” is open to all new and existing customers of New Face MD Miami / New Face MD Orlando / New Face MD Surgery Centers.
  2. To participate, the patient must schedule an appointment and pay for the treatment first, and after the provider performs the procedure, the patient will have one chance to spin the wheel.
  3. The “Bootox spin to win game” will begin on October 15, 2022, and end on October 28, 2022.
  4. Employees of the Company shall not be eligible to participate.
  5. No person may enter any competition hosted partly or in whole by New Face MD on behalf of a third party. All entries must be made personally by the entrant.
  6. No New Face MD staff member directly employed on and off the site, consultant, or anybody directly or indirectly involved in this competition; any spouse, life partner, dependent family member, child, and their life partner or spouse is permitted to participate. This specifically relates to New Face MD Management and staff, service provider staff, managers, and employees are not eligible to enter the “Bootox spin to win game”.
  7. Multiple prizes are not allowed per person.
  8. This special cannot be combined with other discounts or promotions.
  9. If Special coincides with other promotions, the promotion that most benefits the patient will be respected.
  10. All participants 18 years and over, to be eligible to enter the special, must ensure their personal details are submitted.
  11. Participants can only win once on a specific “Bootox spin to win game”. Will not be allowed participants spin the wheel twice on different days.
  12. The game has free prizes, but first, you must schedule an appointment to spin the wheel. In addition, some prizes may require a purchase to be made.
  13. The prize will be redeemed with one of our providers, depending on eligibility. New Face MD will reserve the right to schedule the appointment with the provider that most benefits the patient.
  14. Participants of the Game, following their wheel spin, have a chance to win a prize if the spinner stops spinning and lands on the wheel segment indicating they are eligible to win a prize. Players will win the prize corresponding to the prize stated on that segment on the wheel. Prizes vary on a daily/weekly/monthly basis subject to quotas being claimed and generally include the following:
  • Radio Frequency Full Face (1) Session
  • CO2 Laser Full Face (1) Session.
  • $100 Off Filler or PDO Treatment.
  • Nutritional Plan (30 Days)
  • 50% Off Dental Cleaning and Scanner
  • Laser Hair Removal (1) Small Session
  • 20 Units of Botulinum Toxin
  1. All prizes for all winners are non-exchangeable, non-transferable, and no cash alternative is offered.
  2. The winner can claim the prize by scheduling the appointment until November 20, 2022; the patient will lose the right to claim the prize after this date. Patients from one of our participating clinics will only have one chance to spin the wheel.
  3. Patients from our participating clinics must spin the wheel at least one complete turn.
  4. The appointment may be modified or canceled following the New Face MD, LLC Financial Policy and arbitration agreement. Including the appointment to claim the prize.
  5. In the event of cancellation or no-show, the prize cannot be reinstated.
  6. Prior reservation is required and subject to availability. Blackout dates apply.
  7. New Face MD shall not be responsible for booking requests received after the prize date expiry or for any late, misdirected, lost, or unsuccessful efforts to notify a New Face MD, LLC staff as a consequence of telephone or Internet service delays, interruptions, failures or overloads and shall in no event be responsible for any act, omission, failure or delay arising from, but not limited to, an act of God, riots, acts of terrorism, labor or industrial dispute, any strike, natural disaster, or any other cause beyond the reasonable control of New Face MD, LLC.
  8. New Face MD, LLC reserves the right to amend your appointment in the event any of the terms and conditions is/are violated. In the event of any dispute, the management’s decision is final, and no correspondence will be entertained.
  9. Participants are deemed to have accepted and agreed to be bound by these terms and conditions upon entry and the Company reserves the right to amend these terms and conditions at any time. Any such revised terms and conditions shall have effect immediately and may be found on the Company’s website: com. It is a condition of entry that these terms and conditions are final.
  10. The Company reserves the right to refuse to award any prizes to anyone in breach of these terms and conditions.
  11. The Company reserves the right to hold, void, cancel, suspend, or amend the Game where it becomes necessary to do so.
  12. Any violation or attempt to violate any of the above rules will result in the immediate disqualification of the transgressor.
  13. No financial or other consideration of any nature will be payable to the winner or any family or accompanying party for the possession or utilization of photographs, other likenesses, recordings, or any other advertising material. Insofar as is permitted by law, the Company will not in any circumstances be responsible or liable to compensate the winners or accept any liability for any loss, damage, personal injury, or death occurring as a result of taking up any of the prizes, except where the negligence of the Company causes it. Your statutory rights are not affected.
  14. Personal data supplied during the Game will only be processed as set out in the Company’s Privacy Policy.
  15. The Game will be governed and construed under and in accordance with Florida law. You also acknowledge and agree that the venue and the exclusive proper forum in which to adjudicate any case or controversy arising either directly or indirectly, under or in connection with this special shall be in the court(s) of competent jurisdiction in and for Miami-Dade County, Florida or Orange County, Florida, the State of Florida.
